IMPORTANT NOTICE FOR ALL PATIENTS USING

PATIENT ACCESS FOR ONLINE SERVICES!

 

You may be aware that Sarum House Surgery will be merging with four other Hereford medical practices on the 1st July 2018, to become Hereford Medical Group. Unfortunately, as a result of the merger process all Patient Access on-line services will be unavailable from the 1st May until the 2nd July 2018.

 

The following on-line Patient Access services will be unavailable from 1st May to 2nd July: Ordering repeat prescriptions, booking appointments, sending electronic messages, changing address details and registering as a new patient.

 

Please phone or visit the surgery and our reception team will be able to carry out these tasks for you. To register as a new patient you must visit the surgery.

 

In the meantime, to order repeat prescriptions, please contact the prescription line on 01432 271736 between 10am and 3.30pm each day.  

 

Alternatively, you can email your prescription request to sarum.web@nhs.net 

 

After the 2nd July 2018, patients who wish to use the Patient Access on-line services will be required to re-register for this service, by visiting reception at Sarum House Surgery or at another of the Hereford Medical Group sites with a form of ID. We sincerely apologise for any inconvenience this may cause. We would like to thank you for your understanding and patience during our merger process.

Named GP

Every patient at Sarum House Surgery has a named GP but all of the GPs works closely together as a team and you can choose to see any of the doctors who are available.

 

Please ask at reception if you would like to know who your named GP is.Please ask our Reception team if you would like to know who your named GP is, or if you would like to change your named GP.


 

 

Communication with our patients

We want to get better at communicating with our patients.

 

We want to make sure you can read and understand the information we give you.

 

If you find it hard to read our letters or if you need someone to support you at appointments, please let us know.

 

We want to know if:

  • you need information in Braille, large print or easy read format.
  • You need an interpreter or advocate

We can support you to lip read or use a hearing aid or communication tool.

 

Please tell the receptionist of you need information in a different format or any other support to help you communicate and understand. 


 

 

 

 

 

 

Care Quality Commmission inspection report

The Care Quality Commission recently undertook an inspection of Sarum House. Commenting on inspection, Professor Nigel Sparrow, Senior National GP Advisor said:

 

"It is clear that Sarum House Surgery is providing a caring and effective service for the people of Hereford.  We saw the practice had comprehensive systems for monitoring and maintaining safety as well as the care and treatment of patients. It was proactive in helping people with long term conditions and had arrangements in place to make sure that their health was monitored regularly."
