Surveillance Report- Sarum House Surgery

April to October 2012

 

1.0 Health Economy Surveillance Data

 

The Infection Prevention & Control Team undertakes alert organism surveillance across Herefordshire and for the purpose of this report will be reporting data for Meticillin Resistant Staphylococcal Aureus (MRSA) Infection and colonisation and Clostridium difficile (CDI).   

 

2.0   Vital Signs CDI Trajectory

 

These charts represent the total number of Clostridium difficile (CDI) cases reported on MESS (the national surveillance data base) against the annual trajectory for NHS Herefordshire. The graph further identifies apportioning of cases.

 

2.1   2012/13

 

 

 

 

 

The PCO CDI trajectory is set by the Department of Health. Herefordshire’s annual trajectory is 72 for 2012 /13

The number of CDI cases within Herefordshire 2012/13 reached 30 cases in October against the annual trajectory.

Depending on location and date of sampling, cases are apportioned to the community or Wye Valley Trust (WVT). WVT has its own trajectory of 21 for the County Hospital and 5 for the Community Hospitals. Cases are attributed to WVT or the Community Hospitals if they were identified 3 days after admission (admission day 2 days). If the sample is taken within this timeframe, or in the community, the case will be attributed to the community.

A dual testing algorithm is in use by Herefordshire labs in line with the latest DH Guidance. All samples are primarily tested by PCR and then all PCR ve are retested using the EIA method. Only those testing positive for both are reported in line with DH Guidelines.

2.3 Surveillance Summary for Sarum House Surgery April to October 2012

 

 

 

1 new case of CDI and no cases of MRSA have been identified by patients registered with the practice. This data does not include patients that may have been diagnosed whilst an inpatient at the County Hospital or Ross, Leominster or Bromyard Community Hospitals.

A short root cause analysis (RCA) is undertaken for all cases of community CDI and any issues identified fed back to the appropriate clinicians. Further details of individual cases can be obtained via the Infection Prevention Team.
